1. If you're a medium/reader, using the drop down, select Mediums, (subcategories are optional - In Person and Online Mediums).
2. If you're a church/venue, using the drop down, select Spiritualist Venues (subcategories are optional - In Person or Online Venues).
3. Then click the button 'Load Form'.
4. Complete as much information as possible - *required fields.
5. Once done, click (bottom of page) 'Create'.
